John Desjarlais on autoimmune disease

From Antibodies! Ep. 2: Fc Engineering & Bispecifics with John Desjarlais · · The Antibody Society

“The most disruptive paper that came out in the last couple of years in our field was the Georg Schett paper talking about use of CD19 CAR-T for treatment of autoimmune disease. Right, basically showing leading to a lot of hyperbolic headlines, but basically the concept that treatment with the CD19 targeted CAR-T could effectively put a number of lupus and Sjogren's and myositis patients into remission for long periods of time. Kind of a re-immune reset hypothesis.”

We’re excited to share episode 2 of the Antibodies! podcast, recorded live at the Antibody Engineering & Therapeutics conference in San Diego. Host Dr. Jonathan Davis (  / jonathandavis-bispecifics  )  PhD (Founder, Creative Antibodies) sits down with John Desjarlais (  / johndesjarlais  )  PhD, Chief Scientific Officer at Xencor, to discuss learnings from two decades of antibody engineering: Fc platforms, T-cell engagers in solid tumors, and bispecifics in autoimmune disease. In this conversation, they discuss: • How a classical trombonist ended up engineering some of the most widely used Fc platforms in antibody therapeutics • The KLH study that proved obexelimab could shut down B cells. Data so meaningful, John still keeps the plot on his office wall • Why target selectivity matters more than immune environment for T-cell engagers in solid tumors • The autoimmune gold rush: using T-cell engaging bispecifics to reset the immune system, inspired by Georg Schett’s CAR-T breakthrough • Trispecifics, AND-gate targeting, and where bispecifics can go beyond oncology Be sure to listen all the way to the end for a surprise treat - an improvised harmonica blues duet between Jonathan and John! About John Desjarlais

Executive Vice President of Research & Chief Scientific Officer · XENCOR INC

John Desjarlais, Executive Vice President of Research and Chief Scientific Officer at Xencor, appeared on the Antibodies! podcast recorded at the Antibody Engineering & Therapeutics conference in San Diego. During the conversation, Desjarlais discussed the application of T-cell engager bispecific antibodies in autoimmune disease, citing a paper by Georg Schett on CD19 CAR-T therapy for autoimmune conditions. He described a "gold rush" in deploying T-cell engager bispecific antibodies targeting B-cell antigens such as CD20, CD19, and BCMA, based on the hypothesis that B cells contribute to the pathogenesis of autoimmune diseases. Desjarlais noted that the field is working to determine the optimal depth and durability of B-cell depletion to achieve an "immune reset" while allowing the immune system to recover. Desjarlais also addressed challenges in using T-cell engagers for solid tumors, stating that the biggest challenge is not the immunoenvironment or T-cell presence, but rather the identification of highly selective targets that are overexpressed on tumor cells. He suggested that tri-specific antibodies targeting two tumor antigens could improve selectivity by requiring T cells to kill only when both targets are present, though he acknowledged technical challenges in developing such molecules with good developability and half-life.
